• Severe: hepatitis/liver failure, agranulocytosis, aplastic anaemia, thrombocytopenia, drug fever, lupus-like syndrome, vasculitis, nephritis, interstitial pneumonitis, Stevens-Johnson syndrome. • Boxed warning: severe liver injury and acute liver failure, sometimes fatal or requiring transplantation; reserve for patients who cannot tolerate methimazole and in whom surgery or radioiodine are not appropriate. • May be the preferred agent in the first trimester because methimazole is associated with rare fetal abnormalities; consider switching to methimazole for the 2nd and 3rd trimesters because of maternal hepatotoxicity.
