• Hypersensitivity reactions in about 1.5% of patients (morbilliform eruptions about 1 in 100; pruritus, urticaria and positive Coombs test each under 1 in 200). • Gastrointestinal symptoms in about 2.5% (diarrhoea about 1 in 70); nausea and vomiting rare; pseudomembranous colitis. • Before starting, ask about previous hypersensitivity to cefaclor, cephalosporins, penicillins or other drugs; cross-hypersensitivity among beta-lactams may occur in up to 10% of penicillin-allergic patients. • Cefaclor: Bactericidal; like other cephalosporins it inhibits bacterial cell-wall synthesis. Resistance is mainly via beta-lactamase hydrolysis, altered penicillin-binding proteins and decreased permeability; inactive against methicillin-resistant staphylococci.
