• Most common: dyskinesias (choreiform, dystonic and other involuntary movements) and nausea. • Nonselective MAO inhibitors are contraindicated (stop at least 2 weeks before starting); selective MAO-B inhibitors such as selegiline may be used at the recommended dose. • Stop plain levodopa at least 12 hours before starting. Dyskinesias may occur at lower doses and sooner than with levodopa alone. Observe for depression with suicidal tendencies. • Vitamin B6 deficiency and seizures: treatment may reduce vitamin B6 levels (higher risk with higher doses); refractory seizures resolving only with vitamin B6 have been reported. • Carbidopa: Inhibits peripheral decarboxylation of levodopa and does not cross the blood-brain barrier, increasing levodopa available to the brain.
