• Postmarketing reports and case series indicate that use in pregnancy may increase the risk of fetal adverse effects, including neonatal hypo- and hyperthyroidism, neonatal bradycardia, neurodevelopmental abnormalities, preterm birth and fetal growth restriction; amiodarone and desethylamiodarone cross the placenta. • Amiodarone and desethylamiodarone reach breast milk at 3.5-45% of the maternal weight-adjusted dose; hypothyroidism and bradycardia have been reported in breastfed infants. • Amiodarone: Prolongs the cardiac action potential and refractoriness (class III effect) by blocking myocardial potassium channels; also blocks sodium channels at rapid rates, has non-competitive antisympathetic action and slows nodal conduction (class I, II and IV-like effects).
